Forms and Child Protection Information

Medical Forms

Class 1 and Class 2 Medical Forms. Scouts and adults under the age of 40 may use Class 1 and Class 2 forms. The Class 1 form must be filled out or updated every year by the parent (if for a scout) or adult leader. The Class 2 form must be filled out once every 3 years by a physician.

Class 3 Medical Form. This form is required for all adults age 40 and over. It must be filled out on an annual basis by a physician.

Department of Youngstown/St Paul Church Requirements

Since Troop 35 is chartered with St Paul Church, all adult leaders in the troop are considered volunteers at the parish. As such, we are obligated to follow the guidelines mandated by the Diocese regarding volunteers. These rules apply to anyone over the age of 18 who is involved in Troop 35 activities.

All volunteers are required to:

Volunteers who work directly with children (a child is anyone under the age of 18) are also required to do the following:

• Attend a Diocesean Child Protection In-Service. This 2 to 3 hour seminar is offered periodically at St. Pauls and at other churches in the Diocese. Check the Church bulletin for these seminars. We will also send out periodic announcements via the mailing list.
• Be fingerprinted and have a BCI Report submitted to St Pauls. You can have this done at Walsh University for $20 (call 330-490-7210 for times). You can also talk to Fran Pratt who may be able to arrange the BCI reports through his work.

You may submit forms directly to the rectory, or turn them in to Jim Satrape. If you have any questions, Jim can also probably help you.
