The Ohio Department of Health and the Ohio Department of Education in accordance with Section 3313.671, Ohio Revised Code, have established minimum immunization requirements for pupils enrolled in kindergarten through grade 12 in public or non-public schools. Each school is required by law to have proof that a student has met the state's immunization requirements within fifteen (15) days from the day of enrollment. Exact dates of all immunizations (day, month and year) are needed. If proof of proper immunization is not present or there is no record of medical exemption from immunization then the pupil will be exempt from school until proper forms are obtained. I begin to review the immunization record of each kindergartener, 7th grader and any new student to St. Paul's in August and either notify parents by mail or phone if immunizations are not current. Often times it is simply a matter of the school not having the most recent copy of immunizations. When your child receives a new immunization please ask the physician for a copy of the record and send a copy to the school office so we may update your child's record.
